Malignant Non-Epithelial Tumors of the Ovary - 17 Cases Studies

Abstract:
Malignant non-epithelial tumors of the ovary are rare. They occur in relatively young women. Due to their rarity and heterogeneity, treatment and prognosis still remain a subject of research and controversy. The objective of our work is to analyze the epidemiological, clinical, histological, therapeutic and prognostic characteristics of these tumors in order to provide patients with the best possible care. Our work is a retrospective study of 17 cases of non-epithelial malignant ovarian tumors collected and treated in the GOII department of CHU Fez, from January 2016 to December 2021. The main inclusion criterion was the histological type of the tumor. sexual cord tumors were represented by 11 cases, including adult-type granulosa tumors representing 64% of cases while germinal tumors were represented by 4 cases (23% of cases), and we have compiled a case of Rhabdomyosarcoma ovarian and one unclassified ovarian sarcoma. The clinical symptomatology was dominated by abdominal distension often associated with chronic pelvic pain. The average tumor size was 205 mm in the TGM group and 141 mm in TSCS with a solidocystic appearance on pelvic ultrasound in all cases The dosage of tumor markers (chorionic gonadotropic hormone, lactate dehydrogenase, CA 125, alpha-fetoprotein) was not performed in all patients due to lack of funds. All patients underwent early surgery (exploratory laparotomy) for histological evidence. Non-epithelial ovarian tumors are found most often at an early stage in young women, explaining the high rate of conservative surgery. The prognosis of these tumors has been greatly improved by the introduction of a CT based on cisplatin.
