Trophoblastic Retention on Isthmocele after Pregnancy on Cesarean Scar: A Case Report

Abstract:
Pregnancy on a caesarean section scar is a rare form of ectopic pregnancy of cervico-isthmic and cicatricial location. It corresponds to an implantation of the gestational sac in the scar or dehiscence of the previous caesarean section or at the level of the isthmocoele. It can engage the vital and functional prognosis of the patient by hemorrhage or early uterine rupture. We report the case of an ectopic pregnancy on a caesarean section scar which underwent an incomplete miscarriage with trophoblastic retention on an isthmoceles diagnosed following metrorrhagia in a 34 year old patient, 3rd gesture 2nd pare, carrying a bi-cicatricial uterus. Thanks to endovaginal ultrasound and pelvic MRI the diagnosis was made and the treatment was conservative. Through this observation and in the light of a review of the literature, we will discuss the diagnostic and therapeutic characteristics of this rare entity, the knowledge of which by the practitioners allows to improve the prognosis.
