Intrahepatic Cholestasis of Pregnancy: Case Report and Review of the Literature

Abstract:
Introduction: Gravid intrahepatic cholestasis (CIG) is one of the most common gravid hepatopathies. The disease resolves spontaneously in post-partum; however, it tends to reoffend in a more severe form in 45-90% of subsequent pregnancies. The maternal prognosis is generally good. The main maternal complication is postpartum hemorrhage. The fetal complications make the severity of the disease mainly intra uterine foetal death (IUFD), prematurity, respiratory distress and bradycardia Observation: We report the case of a 35 years old patient, a patient second gesture admitted for the management of a disabling pruritus complicated of a cutaneous jaundice on pregnancy of 31 weeks of amenorrhea, A high level of biliary acid was found in the biological assessment, thus confirming the diagnosis of gravid intrahepatic cholestasis, the course was to establish maternal surveillance: clinical, biological and fetal: electrical and ultrasound, the evolution was marked by a premature delivery at 32SA of a newborn with a severe respiratory distress and neonatal death. Conclusion: the CIG which remains the most common gravid hepatopathies to evoke before pruritus appearing late during pregnancy requiring special monitoring given the consequent fetal and maternal complications
