Les carcinomes mucineux du sein : à propos de 14 cas

Abstract:
Purpose : Mucinous carcinoma of the breast constitutes 1 to 7% of all breast cancers. There are two histological subtypes distinguished: mixed mucinous carcinoma, where the carcinomatous and mucinous component are entangled, and pure mucous carcinoma with favorable prognosis, where the mucus surrounds the tumor tissue and thus constitutes a mechanical barrier attenuating cell invasion, making this form less aggressive. The objective of the study is to retrospectively specify the epidemiological, clinical, biological, therapeutic and prognosis peculiarity of this rare form of breast cancer. In this work, we propose to study first the clinical, anatomopathological and progressive characteristics of 14 cases of mucinous carcinomas, secondly, we propose to identify through a review of the recent literature, the management therapeutic of these carcinomas. Methods: This is a retrospective study of 14 cases of mucinous carcinoma, collected over a period of 4 years (March 2016- March 2020). Clinical, radiological, anatomopathological data were collected from medical records and MCM files Results: The average age of our patients was 61 years (38-80 years). The tumor was unique in 13 cases, and bifocal in one case. The mammography objectified an ACR4 lesion in 50% (7 cases), ACR5 in 28.57% (4 cases) and 21.43% (3 cases) suspected diagnosed on CT. The mixed form is found in 9 cases and the pure form in 5 cases. Lymph node involvement was noted in 3 patients. The number of metastatic nodes varied from 2 to 9 with an average of 2. Hormone receptors were positive in 13 tumors (93%). HER 2 was overexpressed in one patient. Surgical treatment was radical in eight patients (57%), conservative treatment 1 case, cleanliness mastectomy for 1 metastatic case, 3 cases of neoadjuvant chemotherapy, 1 case of axillary dissection only (patient benefited from a lumpectomy only then contact us as a supplement to the treatment), The overall 5-year survival was 100%. Conclusion: Mucinous carcinoma comes in two distinct pure and mixed types with different prognosis A larger sampling and a longer follow-up are necessary to better understand this particular entity
