Genital Tuberculosis In Women In Its Pseuotumoral Form, Mimicking A Latero-Uterine Anexial Mass Of Infectious Or Tumoral Origin: About 04 Cases

Abstract:
: Genital tuberculosis is rare. The lack of clinical specificity makes it difficult to identify this infection and the diagnosis is often delayed. We report 04 cases from our department, consulting for various reasons such as abdominal enlargement with deterioration of the general state for 03 cases, and pelvic pain, in a context of weight loss for 01 cases. The radiological presentation was not specific. The results of the radiological explorations (hysterosalpingography, pelvic ultrasound associated with pelvic CT or MRI) were in favour of tumorous ovaries, with peritoneal carcinosis in three cases, and of a tubo-ovarian abscess in 1 case. The diagnosis was made by anatomopathological examination of a peritoneal and omentum biopsy in 2 cases, the product of biopsy curettage of the endometrium in 2 cases. In all cases, the anatomopathological study revealed gigantocellular granulomas and caseous necroses, which are pathognomonic lesions of tuberculosis. Tuberculosis should be suspected and investigated in any woman with secondary amenorrhea, but it should be considered as part of the primary infertility work-up, particularly in endemic countries, as it can mimic a malignant tumor, most often ovarian with peritoneal carcinosis, in its pelvic pseudotumor forms.
