Abstract:
Fournier's gangrene is a rapidly progressive necrotizing fascist of the perineum and external genitalia. It is secondary to a polymicrobial infection by aerobic and anaerobic bacteria having a synergetic action. The source of infection is cutaneous, urogenital or colorectal. Contributing factors, such as age, diabetes and immunosuppression, or local infections such as bartholinitis are often present in affected patients. , Urgent and surgical treatment is essential to ensure the survival of the patient. Hydro-electrolyte rebalancing, balancing diabetes, a high-protein diet with broad-spectrum antibiotic therapy quickly followed by surgical debridement, is the basis of treatment, to ensure better healing. However, mortality remains high, around 20-80%, often due to delayed diagnosis and management or rapid spread of infection resulting in septic shock Patients who survive infection will sometimes undergo recovery surgery with sequelae sometimes very marked in relation to the extent of fasciitis and debridement
