Epidemiological, Diagnostic and Therapeutic Profile of Both Cases

Abstract:
Adnexal torsion is a relatively frequent pathology. It represents 2.5 to 7.4% of women consulting for acute pelvic pain (1). Torsion is defined by the existence of a rotation of at least one turn of the appendix on its axis, which is the lumbo-ovarian ligament and the tubo-ovarian ligament. We present 2 different cases of adnexal torsion in two women who consulted for acute latero-pelvic pain. Both patients underwent exploratory laparotomy (lack of laparoscopy in the emergency department) returned in favor of torsion of the appendix on pregnancy in the 1st trimester in good condition for the 1st case, and torsion on a pathological ovary in severe ischemia for the 2nd case. We carried out an initial detorsion for the 2 cases, the surgical treatment was a conservative treatment by performing a simple cystectomy without rupture for the 1st case and a radical treatment made of an adnexectomy + peritoneal biopsies + cytology for the 2nd. The post-operative follow-up was unremarkable. The operating parts were sent to the anatomopathology department and an appointment in 3 weeks was fixed. The torsion of the adnexa is an emergency not to be ignored in the face of any acute pelvic pain in women, regardless of their age. Conservative treatment is currently the gold standard and appropriate management is necessary to avoid possible maternal and fetal complications. In case of pregnancy
