Abstract:
: Antenatal aneurysmal dilatation of the intra-abdominal umbilical vein (AODV) is a rare pathology of the fetal venous system. Its diagnosis is based on color Doppler ultrasound. Its discovery requires the performance of a complete fetal morphological assessment in order to eliminate the presence of other lesions, associated forms being found in 29 to 35% of cases. The most feared complication is the occurrence of intrauterine fetal death (IMF). When isolated, it does not justify modifying obstetric care. However, ultrasound monitoring remains recommended during the third trimester to look for increased dilation and especially the appearance of a thrombus. A particular clinical form, linked to an abnormal entry of the umbilical vein into the infra-hepatic portal system, must be individualized due to a high risk of thrombosis. It justifies carrying out an abdominal ultrasound in the first days of life. We report here a case of antenatal aneurysmal dilatation of the umbilical vein discovered incidentally during an ultrasound at term. The pregnancy proceeded without incident with a spontaneous vaginal delivery of a newborn in good condition. health. Postnatal ultrasound revealed an intrahepatic arterioportal fistula with hepatic perfusion disorders.
