Abstract:
: "Fibroids are the most common benign pelvic tumors in women, with a prevalence in the general population ranging from 20 to 50% of women of childbearing age [1]. During pregnancy, the frequency of fibroids is estimated to be between 3 and 13% [1]. Their discovery in this context is even more common as the age of patients at the time of their first pregnancy increases [2], and the average number of ultrasounds performed during pregnancy has been increasing for the past 20 years [3]. The effects of fibroids on pregnancy and delivery, as well as their management, especially in the case of cesarean section, remain controversial over 100 years after Victor Bonney's first description of a per-cesarean myomectomy [4]. Updated clinical practice recommendations in 2011 [5] acknowledge an increased rate of obstetrical complications in cases of fibroids but do not recommend myomectomy during pregnancy or during cesarean section, unless it is necessary or justified (such as in the case of praevia, for example). Recent studies allow for a nuanced view of this approach [6-10]. Here, we report the case of a patient who presented to the emergency room for a full-term labor, and the patient is known to have a fibroid. During the cesarean section, a 30 cm fibroid with a type 7 pedicle was discovered, leading to the performance of a myomectomy."
