Abstract:
Paget's disease of the nipple is a rare pathology associated with 1-4% of breast cancers. It corresponds to infiltration of the nipple epidermis by adenocarcinoma-type cells. Clinical symptoms include eczematous eruptions on the nipple and areola. Paget's disease is associated with homolateral breast cancer in 82 to 100% of cases. Imaging is based on breast ultrasound, with mammography and breast MRI playing a vital role in the search for underlying cancer. Paget's disease is diagnosed with certainty either by cytological scraping of the nipple or, at best, by areolomamellar biopsy. The management of Paget's disease, for which there is as yet no consensus, ranges from radical treatment to removal of the areolomammary plaque (pamectomy), followed by radiation therapy. Conservative treatment depends on the associated breast cancer. The prognosis of Paget's disease of the nipple depends on the presence of an associated palpable lesion and the re-invasiveness of the underlying cancer.
