: Phyllodes tumors are fibroepithelial tumors, developing at the level of epithelial cells and connective tissue. In the vast majority of cases, they are benign tumors, but there are also tumors called restricted malignancy or intermediate risk, which can sometimes develop into cancer. High-risk phyllodes tumors are called phyllodes sarcomas: they can lead to the formation of metastases in some cases. Phyllodes sarcomas of the breast are rare tumors and represent less than 1% of malignant breast tumors. Their clinical and radiological characteristics are those of benign tumors, and only rapid tumor growth can suggest the diagnosis. Their treatment is based on surgery, chemotherapy and radiotherapy, with a good prognosis. We report the case of phyllodes sarcoma of the breast with pulmonary metastasis managed in our department. The clinical diagnosis was made in view of the rapid evolution of the tumor and confirmed by the anatomopathological examination of the surgical specimen. The extension assessment highlights a pulmonary metastasis the patient underwent a mastectomy with axillary visit then referred to oncology for chemotherapy.
