Title: Amyand's Hernia in an Infant: A Rare Intraoperative Diagnosis of Inflamed Appendix in a Congenital Inguinal Hernia - A Case Report

Abstract:
Background: Amyand's hernia is a rare form of inguinal hernia in which the appendix is located within the hernia sac. In pediatric patients, its preoperative diagnosis is particularly challenging due to its rarity and the similarity of its presentation to typical inguinal hernias. Case presentation: An 1 8-month-old boy presented with a reducible right inguinal swelling and was diagnosed clinically with a congenital inguinal hernia. Intraoperatively, an inflamed appendix was found within the hernia sac, confirming a diagnosis of Amyand's hernia (Losanoff and Basson type 2). Appendicectomy and herniotomy were performed successfully. The patient recovered well and was discharged in good condition. Discussion: Amyand's hernia is extremely rare in children, representing approximately 1 % of inguinal hernias. Preoperative diagnosis is seldom made, and most cases are identified during surge ry. This case highlights the role of intraoperative vigilance and supports the Losanoff and Basson classification for guiding management. Conclusion: Prompt recognition and appropriate surgical intervention lead to excellent outcomes in pediatric Amyand's hernia.
