Abstract:
We report a rare case of alveolar rhabdomyosarcoma (ARMS) of the prostate in a 23-year-old male presenting with obstructive lower urinary tract symptoms, gross hematuria, anorexia, and systemic inflammatory signs. Computed tomography and magnetic resonance imaging revealed a massively enlarged, heterogeneous prostate with a 25 × 18 mm lesion breaching the capsule, along with extensive retroperitoneal, pelvic, and inguinal lymphadenopathy causing left ureteral compression and minimal hydronephrosis. A full-body scan showed no evidence of distant metastasis. Urine analysis demonstrated marked leukocyturia and hematuria, while cultures remained sterile, indicating a cyto-bacteriological dissociation. Histopathological examination of a prostate biopsy confirmed ARMS with immunohistochemical positivity for desmin and myogenin. Informed consent for publication was obtained, and the patient is scheduled to initiate chemotherapy this week as part of a multimodal treatment strategy that includes planned radiotherapy and potential surgical intervention. This case underscores the diagnostic challenges of prostatic ARMS in young adults and highlights the need for early, aggressive multimodal therapy.
