Abstract:
Acute scrotal pain is a urological emergency requiring immediate evaluation. Testicular torsion predominantly affects neonates and adolescents, while it is rare in individuals over 40 years of age. We report a case of a 50-year-old male with acute right scrotal pain of 48 hours' duration. Scrotal ultrasonography revealed an avascular, enlarged, and heterogeneous right testis, suggestive of torsion with ischemia. Surgical exploration confirmed complete testicular necrosis due to torsion, necessitating orchiectomy. This case highlights the importance of maintaining a high index of suspicion for testicular torsion, even in older patients, and underscores the need for prompt surgical intervention.
