Title: Gastric Volvulus Induced by Wandering Spleen Leading to Gastric Outlet Obstruction: A Case Report and Literature Review

Abstract:
Wandering spleen is a rare condition characterized by abnormal splenic mobility due to laxity or absence of its supporting ligaments. Complications may include torsion, infarction, and mechanical compression of adjacent organs, potentially leading to gastric volvulus and gastric outlet obstruction. A 58-year-old male presented with acute abdominal pain, distension, and vomiting. Imaging revealed a massively enlarged, ectopic spleen (25 cm) extending into the pelvis, compressing the duodenum and causing gastric distension with concurrent gastric volvulus. Emergency laparotomy was performed, involving gastric de-volvulation, reduction of the left colon, and anterior gastropexy. The spleen was repositioned without splenopexy or splenectomy due to preserved viability. This case underscores the importance of considering wandering spleen in the differential diagnosis of gastric outlet obstruction and highlights the necessity of prompt surgical intervention to prevent life-threatening complications.
