Abstract:
:Introduction : Infectious sacroiliitis represents 1 to 5% of septic arthritis and 1.5% of septic arthritis in children. It is a rare condition with a deceptive clinical polymorphism making diagnosis challenging. The treatment is theoretically simple, effective, and the prognosis is favorable. We report through an observation the diagnostic difficulties and consider the various clinical aspects of infectious sacroiliitis, the diagnostic means, and the therapeutic strategy to be adopted. Observation : Patient aged 17, without pathological history, admitted for total functional impairment of the right lower limb for 4 days without fever, with a positive infectious assessment. The diagnosis of septic arthritis of the hip was initially suspected. The clinical (no fever), biological (CRP at 260 mg/l, GB at 22000 mmol/l), and radiological (standard X-ray and ultrasound of the hip and abdomen normal) discordance led to further investigations with clinical reassessment. The gentle examination revealed a free hip with limited pain near the sacroiliac joint and the right buttock. A pelvic CT scan was then performed, favoring a right sacroiliitis. The patient evolved well under dual antibiotic therapy for 3 weeks and at the 6-month follow-up, he walks unassisted, without joint limitation or pain, with normal resumption of sports..Conclusion :Infectious sacroiliitis is a rare condition in adolescents. The clinical presentation is vague and nonspecific. When diagnosed early, the prognosis is very good with antibiotic therapy. It must be kept in mind in the presence of any acute functional impairment.
